The following documents are required to complete our review and prepare the Mechanics’ Lien Statement; (i) Copy of the complete signed contract and all attachments; (ii) Copy of all signed and unsigned Change Orders; (iii) Copy of all invoices submitted to the customer; (iv) Copy of all payments received from customer; (v) Statement showing Invoices sent minus payments received and balance remaining (the balance remaining should match the amount claimed above); (vi) documentation proving first day of contribution (time card, delivery ticket, etc.); (vii) documentation proving last day of work (time card, subcontractor invoice, etc.).

    MNCLS cannot guaranty that the Mechanics’ Lien Statement will be filed on time if this intake form and the required supporting documents are received more than 110 days after your last day of work.
